Your security is our first priority. We need your permission to use electronic medical information and telecommunications technologies to support and promote long-distance clinical health care.

Enter your information and find your profile
Using your name, phone number and date of birth, we'll be able to find your patient record in our medical record system.

Set a username
Choose a username for your account. This username should have a minimum of 6 characters. Your username will be used to log in the next time you use the app.

Download and install the app
After installing the app on your device, open it and follow the link for "First Time Sign Up".

Select the practice you belong to
As a miinedoctor patient, simply select that practice and hit next.
Step 6
Verify your contact information
For security purposes, we need to verify your mobile device. We'll text a 6-digit code to the phone number on file. Simply enter that 6-digit code to verify.
Step 8
Set a password
Select and set a password for your account. This password should have a minimum of 8 characters. This password will be used to log in the next time you come back to the app.
